Application forms can be obtained from the Examination Yuan by fax, or by mail, if you send in a stamped, self-addressed envelope. |

The forms may also be downloaded from the Examination Yuan website and printed. (Please print on A4 paper only). |

於填妥申請表後，檢齊附件，向考試院申請辦理。 |
Once filled out, any necessary attachments should be enclosed, and the forms then forwarded to the Examination Yuan for processing. |

申請人申請補發考試及格證書、改註作業，若有急需，可親自或委託他人 ( 須檢附委託書 ) 來考試院辦理，隨到隨辦，一小時內辦妥。 |
Persons who urgently require replacement of or amendments to a certificate may go to the Examination Yuan in person where the matter can be handled within an hour. (If you have someone else do this for you, you must give that person a letter of authorization). |
